Cancellation/Late Arrival Policy

If you are unable to keep your appointment, we kindly request that you contact our office at least 24 hours before your scheduled appointment so we may offer that appointment time to another patient.  A fee will be charged for any no call/no show occurrences, cancellations without 24-hour advance notice, and late arrivals [twenty (20) or more minutes following your scheduled appointment time].  See fees below:

-First (1^st) and second (2^nd) Late Cancellation/Late Arrival: $10

-Third (3^rd) and any additional Late Cancellation/Late Arrival: $25

-ALL missed appointments considered a no-call/no-show: $25

Patient Financial Responsibility

It is unlawful for healthcare providers to routinely waive copayments, deductibles, coinsurances, or other patient responsibility payments.  Failure to comply is a violation of the Federal False Claims Act, Federal Anti-Kickback Statute, and the Federal and State Insurance Fraud Laws.  Therefore, all patient responsibility payments (co-pays, co-insurances, deductibles) are due at the time of each physical therapy appointment.
